Top of my head, if it's a title match, the champion is conscious, his opponent is outside, the champion has the right to stop the count out if he wishes. Heels wouldn't stop the counts, babyfaces would.
I'd love to see the World Champion have more control. The commentators and wrestlers always comments on about how important it is to be champion, but we rarely see it. Something along the lines of a champion's choice of who they defend the title against with the options being the top 3-5 stars of the brand. Other things could include refusing to defend the title on free TV. For example, Randy Orton has defended the world title within thirty days, so he refuses to defend the WWE title against Triple H on Raw. So either the match is off or the general manager must turn it to a non-title match. Something small like that could do a lot to capture heel heat. You could even make the opponent's choice match that would precede a PPV match on free TV become a regular legal event. You will give Orton one time to create his opponents match before every PPV title defense.
Any sort of rule where being World Champion actually has an advantage other than having a belt and main event glory.

I'd have it slightly different and instead of a count out have it for a DQ which the NWA has done for quite some time and is why Abyss won the title from Sting a few years ago.

Didn't he get disqualified for pushing the referee during a barbed wired match?
Something new would be if referees actually enforced the rules properly:
No ten count in triple threats
Closed fists lead to disqualification
Only legal men can win tag matches
No getting up on the top rope. Ever.
Holds broken immediately once contact with the ropes is made

If i remember, it was a normal match between the two and Sting knocked Abyss off the ramp into a barbed wire table which caused the DQ and thats when Abyss won.
I think Sting actually took a chair to Abyss to try and keep him down which caused the DQ.
One rule i'd like to see is in Multi man matches the title only changes hands if the champion gets pinned. A lot of people complain about title's changing without the champion losing, this would cut that out.
