What is the most difficult and challenging test you've ever taken?
When I was in my first year of junior college (I'm in my last year of university now) I had a history teacher who would give us multiple choice essay exams. He would teach the whole lesson and then at the end of the chapters give you a random topic on the chapter and have you write a four page essay on it before the class ended.
I must say I struggled hard on those exams and pulled out all strong B grades.
What's your experience?

Driving test. I know strictly speaking it's not a written exam or anything, but I failed my practical driving test SIX times. I just couldn't get that bastard.

I got like 35% on a Math test in Grade 8. But to be fair, the highest mark was something like 51%, and we're all pretty sure that most of the questions didn't have real answers.
My Political Science final last semester was pretty brutal.
Final exam when I was finishing up my Master's Degree. No open book and had all technical linguistic stuff to remember and write an essay about. Still to this day don't understand it.
